Sentiment: How much are you willing to automate verse check by hand? To be fair, I’ve only included those I’ve actually used (Radian6, Sysomos, Spiral 16) or tested (Meltwater, SM2). I'm currently using SM2 for most of my clients, and like how easy it is to use. In other words, how accurate does it need to be?

I mirror your sentiments. And then there are monitoring tools like Alterian's SM2 that are out of most people's price ranges. The problem with the approach most people take is how to quickly garnish all attention to surround them, without thinking about a holistic long term approach.
